I'm still confused. What category should I be using? What exactly
do I enter in that box? Template? Infobox? Neither of those do
anything.

You can leave the input box blank. It is a convenience for filling pages
from categories, so you write there a category
Sure, but perhaps it doesn't meet the requirements for an extension? e.g.
certain variables, functions or methods may be expected to be provided.

I did manage to fix it with the help from the users on the mailing
list. It was an issue with the ftp binary/ascii mode transfer which
was spoiling localsettings.php
