No. It means that the insurance company paid off on a car rather than fix it.
Could be a wreck, and often is. Could be hail damage, paint keying...

Not necessarily, it could be for any number of things up to a fairly
significant crash. A carfax would show some insights.
This car probably has a bad engine. Check the vin to be sure.

Would be nice if sellers of rebuilt cars would at least address WHY the car has
a rebuilt title, i.e. what was damaged and repaired.
