[MBZ] A critical view of current Covid-19 testing

2020-05-27 Thread archer75--- via Mercedes
From the business magazine Financial Times (FT):

Like countless others, I was convinced I’d already had coronavirus. I travelled
a lot at the start of the year: a hellish, delayed, 30-hour flight to the UK
from Australia, where I had spent Christmas and New Year, a skiing trip with
friends in the Swiss Alps, and a week at the World Economic Forum in Davos,
where I shook hundreds of hands in stuffy conference rooms by day and at
crammed cocktail parties by night.

In February, I developed the worst chest infection of my life, so persistent
that one night, while eating in a restaurant, I had to go home mid-meal because
I could not stop coughing. I discounted any evidence that contradicted my
hypothesis: at no point had I developed a noticeable fever, my sense of taste
and smell had remained intact, and I did not seem to have passed the illness to
family, friends or anyone at work.

When a test that could apparently prove I had contracted coronavirus first
became commercially available in New York last month, I asked my doctor to send
me for a blood draw. Rather than testing whether a person has an active
infection, these tests perform a sort of biological archaeology, searching for
pathogen-fighting proteins known as antibodies that show a person has had the
illness in the past and possibly developed enough immunity to fend it off in
the future.

The story of antibody testing thus far is a microcosm of the broader response
to coronavirus itself: misplaced confidence and hype followed by premature
pessimism. In March, the UK government announced it would make millions of
at-home antibody tests available via Amazon and in high-street pharmacies.
Those who tested positive would be given “immunity passports”, enabling them to
re-enter the workforce and everyday life. The Chinese-made tests — procured at
a cost of £16m — turned out to be so unreliable that they never made it on to
the market. In the US, the Food and Drug Administration allowed more than 100
different tests to go on sale, before performing a U-turn and warning that some
“unscrupulous actors” had been peddling fraudulent kits.

Last month, an alarmed World Health Organization said there was “no evidence”
that such tests could “show that an individual is immune or protected from
reinfection”. Later, it partially walked back those comments, adding: “We
expect that most people who are infected with Covid-19 will develop an antibody
response that will provide some level of protection.”

The allure of such a test is obvious: a positive result might be a one-way
ticket to a semblance of normality. Underpinning the initial fanfare was a
misguided hope that a large chunk of the population had contracted coronavirus
but experienced only mild-to-moderate symptoms or none at all. If that were the
case, society might be closer to “herd immunity”, which generally means a
combined infection and vaccination rate of between 60 and 80 per cent, and a
return to the quotidian would have been just around the corner.

It’s like a war zone where you use B-52 bombers to fire on a tiny village
with only a hundred enemy people hiding there
Prakash Nagarkatti, a professor in the pathology, microbiology and
immunology department at the University of South Carolina

Such a reality has not come to pass. Even in New York state, on many measures
the global centre of the virus, only 12.3 per cent of the general population
has developed antibodies specific to this particular strand of coronavirus,
Sars-Cov-2, according to a recent study of 15,000 people.

“The hope was that we’d roll out the testing, find lots of people with
antibodies, infer many people are immune, and we can start going back to our
regular lives,” says Lloyd Minor, dean of Stanford University’s school of
medicine. “By anyone’s measure, we’re nowhere close to that.”

While the early hype was misplaced, there is a risk that the pendulum has swung
too far the other way. A colleague of mine recently got tested for antibodies,
but her doctor said even a positive result was of little use: she was told she
was still at risk of reinfection or could catch a mutation of coronavirus.
However, there are very few if any documented cases of reinfection in the
medical literature; and, while coronavirus is certainly mutating, most
scientists say it is not yet doing so at a pace that would mean humans with the
right level of antibodies are unable to fend it off.

My own test took place at a privately run centre in Harlem, operated by Quest
Diagnostics, one of the biggest laboratory operators in the US. My blood sample
was then shuttled 200 miles away to Baltimore, where the plasma was spun off
and tested on a platform developed by Abbott Laboratories, one of the world’s
largest diagnostics companies. In the US, a dozen similar tests have been
approved for emergency use, including one made by Roche, the Swiss
pharmaceuticals group, as well as less well-known

I am sure it is not working.  You are right there is very little
resistance in the pedal which my other 240D has, this one has zero.  Can
I pull the cylinder out without disconnecting the line and push the
pedal to see if the piston comes out or not?  I am sure this is all
basic stuff but I have never messed with clutches before.

Re: [MBZ] Meat

2020-05-27 Thread Dan Penoff via Mercedes
We have a local farmer’s market/restaurant supply place that opens at 4:00 am
during the week who carried bulk meats. I can buy a whole beef tenderloin
(filet) for about $30 there. A whole pork tenderloin under $20. They have all
sorts of bulk meats and it’s cheap as wine. A big produce section with just
about any kind of produce you can imagine, too. You don’t have to buy produce
in large lots, they sell most of it by the individual item or by weight.

When everyone was freaking out about a shortage of meat I went there early one
morning just to see what they had - plenty of meat, so I stocked up.

[MBZ] ***SPAM*** RE: OT plumbing CPVC

2020-05-27 Thread Greg Fiorentino via Mercedes
Shut off the main supply and replace the house main valve? Open and shut the
valve in the possibility there's crud preventing the valve from shutting
completely?

Greg, not a plumbing expert, but it seems logical.

Subject: [MBZ] OT plumbing CPVC

The fun is never-ending. Came down to the basement this morning to find
water covering the floor. Traced it back to the water heater. OK, it's
22 years old so not too surprising.

Picked up a replacement at Lowes. The plumbing in the house is all CPVC,
so I need to glue up a couple of new pieces to connect the new
heater. Problem is that being at the low point of the house, there's a
never-ending slow drip coming out of the cut-off end supply
line. Possibly the house main valve isn't making a complete seal. Anyway
I'm finding it impossible to keep the end of the pipe dry to apply the
cement.

I recall hearing about a trick of using bread to block a slow drip in a
copper pipe before sweating a joint together. Thinking that might work
just as well here. My other idea was toilet paper. It disintegrates when
wet so it should flush out cleanly once the water is back on, I would
think. Any other ideas?

Be sure to get the “hot” PVC glue that sets up quickly. If you apply it
properly, once the fittings are together water won’t affect any cement that’s
there unless it’s under pressure.

Re: [MBZ] ***SPAM*** RE: Meat

2020-05-27 Thread Dan Penoff via Mercedes
My mother in law lives just a mile or so from the Gulf coast, and on any given
day along some of the major thoroughfares around her you can find fishermen
selling fresh fish, shrimp and stone crabs when they’re in season.

If we go over there for a day’s visit we’ll often buy a couple pounds of fresh
shrimp and have them grilled for dinner. Yum.

With the current prices of meat at the store with ground beef being unavailable
or over $8 per pound, streaks being much more that that I decided to explore

I did in the mean time is find a local wholesale meat supply place that
supplies meat to restaurants. I bought 40 lbs of ground beef, 10lbs of beef

There are 2 issues here. We all want hospitals to survive financially, but we
don't want a perverse incentive to degrade the data we rely upon to drive
policy. There should have been some other mechanism to make sure hospitals

Suffice it to say this is all very complex and a ton of admin expense
results from the complexity.

Basically hospitals make all their money on outpatient surgery. My hospital
loses money on most medicare admissions, so an extra 20% on pneumonia
admissions isn't going to go very far when all outpatient surgery cases
were cancelled for two months and there are increased costs in material and
staffing for all hospitals during this outbreak. We do maybe 2000 spine
surgeries a year here. We have around a hundred covid patients on avg over
the past two months. There are lots of non medicare patients to whom
increased reimbursement would not apply. We see many unfunded homeless etc
patients and we try to get them emergency medi CAL but much of it the
hospital just eats. Any emergency funding for that population would be
appropriate when the hospital is losing so much money. I dont know how
many millions we are in the hole so far but it is not a good time to be a
hospital financially. How all the bailout funds will be allocated is above
my pay grade. Anesthesiologists and surgeons all took a major hit in the
wallet. Anybody in private practice has had a very challenging couple of
months with all elective care shut down and people avoiding coming in. We
are doing a lot of telemedicine visits which helps but is no substitute for
a lot of patients.

The Abbott machines are point of care and single test bandwidth. Minimal
impact on overall case numbers.

Most tests go to labs where larger machines run them in batches. That is
how we can do 300k/d or whatever we are doing.

Re: [MBZ] Crack in radiator

2020-05-27 Thread G Mann via Mercedes
Having mastered the skill of plastic welding to my satisfaction, the
suggestion and question raises.
"Could this material be welded?"

Successful plastic welding has a "First Rule" which is : Identify the type
of plastic and match the filler rod with the base material to be welded.

Since the radiator tank is a thermo molded plastic, that narrows the
selection field, somewhat. Also, somewhere on the part, there will be a set
of symbols which identify the plastic type...
Armed with that, and proper pre cleaning, and good technique, you should be
able to effect a repair that would hold.
All that said.. still better to replace the cracked tank.. or the
radiator... If it has cracked once, there are likely other internal
stresses in the plastic from repeated heat cycles and it has ended it's
useful life.

Re: [MBZ] OT Wuhan Red Death: Can't trust the numbers

My understanding is that the machines can be used for many different tests. I
don't think it is the machine itself that contributes to the unreliability of
the infection test.

Abbott Labs test machines are turning out to be very unreliable, so the
infection numbers are inaccurate.

Then we have racial disparity related to infections and outcomes.

https://jamanetwork.com/journals/jama/fullarticle/2766098

Looks like you are poor, have too much family living in a small space, so you
all get ill.

But you will not be able to get medical attention because the hospital is
tossing over 5500 employees out the door.

https://www.kiro7.com/news/local/uw-medicine-announces-furloughs-4000-workers/XRY24ATJPFDBHAVOM5IF3JLZRM/

And that makes womyn the victims, because they are losing their jobs and
childcare. Dames are in the lower income entertainment and hospitality jobs
that are just GONE. If your restaurant, bar, theatre, hospital, dental office,
office of any kind is out of business or closed up until it is “safe”, you are
looking for welfare.

But that is ok, because “The State” will securely provide relief funds. Oh,
wait, they hired morons.

Tank doesn't sit level and therefore isn't a constant shape. Dad's Jeep
Liberty was notorious for this, at 1/4 tank you need to look for gas NOW, its
got like 1/16 range left.

There used to be a kit with a close-fitting metal sleeve that was glued on the
