My insurance company (Geico) was very helpful after I backed my GLK 250 SUV
into a Fedex truck.  I was able to choose my own body shop and when the
repairs to the rear bumper proved more costly than the initial insurance
company appraisal, they approved an extra $1k.

I actually had, about $1000 dollars left after I bought the car back and made 
full repairs. Junk yard replacement parts which the insurance company would't 
permit? Insurance company was AAA at the time. Will see if I can find a photo 
of the damage. I remember having to get the car inspected after the work was 
completed and had to provide receipts of parts bought and repair work.

Exactly.
A good body shop will have a full body frame machine with lasers to give
precise alignment of body alignment points, and that data exists for every
car made and sold in the world now.
The new frame machines are designed to "pull" unibodys back into alignment.
They are not cheap, and the shops charge accordingly, but new and late
model cars are now not cheap either. "B" pillars are a replacement item,
believe it or not.. should it be damaged to that point. It's easy, just
bring money ;))

I went out and looked and at first glance it just looks like door damage. 
However that gab at the top indicates much worse issues. The old woman did hit 
the car pretty hard and I’m betting it did actually tweak the b pillar. I will 
see what the estimates say but my inclination is to have it fixed 
professionally. If it does have b pillar damage I bet that can get expensive so 
I still bet they end up trying to total it. If that ends up happening it would 
be a real shame. At that point I would have to decide if I go ahead and swap

From my experience, take a very serious look at the "B" pillar. If it is
bent [which is likely, based on the pax door damage] B pillar damage will
show up in a change in the door margins of the rear door.
A good body shop would catch it on inspection... a schlock shop used by the
lowball insurance would not.
You can check it yourself with a set of vernier calipers by measuring the
door gap margins on the driver side in multiple places, then take the same
measurements in the same locations on the damaged pax side..
Make up a work sheet with a door gap drawing and R vs L side written
measurements... and hand it to the insurance appraiser... "Look at this!"
"I want it fixed right"

Oh I am. I have had probably 5 different people calling me about setting up to 
take it to a shop. The shop I told them it was going to was the shop the dealer 
uses and is MB certified. The last guy that called started yapping about how 
they charge too much and it might be more than their policy pays. I informed 
them that I am not concerned about their policy and their insured hit my car 
and I am only concerned with being made whole.

When my Dodge pickup was totaled, USAA didn't fuss at all selling it back to me.

A complete Cummins 12-valve 6BTA drivetrain for $1000 was a very good deal.
The total, to put it back on the road, was roughly $7500, which included 
brand-new
wheels and tires all around.  Leaving some $12000 'profit' from the settlement.
Dumb-asses...  I didn't want _money_, I just wanted my truck back.

When my 1994 S420 got totaled out by Allstate they wouldn’t let me buy it back. 
They said it could be done but approval had to go up the chain a couple of 
levels and was likely to be denied. Not sure if that is an across the board 
policy for them or if it’s a case-by-case thing.

about 70% of the value of the car, they just write it off. Any more than that, 
and they are afraid that additional damage discovered after the work begins 
will have the total claim exceeding the value of the car.

wouldn't let me get it repaired. I bought it from them and then repaired and
