I added the fallback MX host line and mail is flowing properly.

Your config file seems okay, are you _sure_ sendmail is currently using
the right one ? You can check it in /var/run/sendmail.pid

I have a problem on 4.2 when sending mail via a smarthost.
Basically the DS host is not being used.
