dead_vnops.c exports a variable dead_vops which is used in vclean() as the
operation vector for a vnode that can no longer be used, such as after
revoke(2) or when the filesystem has been forcibly unmounted.  Providing a
dummy vector like that avoids adding checks to each VOP_* call.

fifo_vnops.c provides the functions used in the struct vops vectors for
FIFOs on all filesystem, providing the actual behavior of FIFOs everywhere.
