I've got:

set alternates=(a@domain|b@domain|c@domain|postmaster@domain|etc@domain) 
set reverse_name
folder-hook .       unmy_hdr From:
folder-hook a       my_hdr From: a@domain
folder-hook b       my_hdr From: b@domain

...and it works beautifully, as long as the To: address on the message
I'm replying to is part of my 'alternates'.

Do you have the unmy_hdr From: folder hook there, and all the
appropriate addresses matched by your alternates?
