I'm thinking something like the link between the brake drum and the bike
frame that attaches with a small bolt and cotter pin. If I'm thinking about
this correctly, if that rod wasn't there (missing cotter pin = bolt backs
out = rod falls off), the hub would spin and lock up the rear tire...

Ok, thanks for the extra information. Right now the only things I can think
of are tire pressure drop or tire failure (which would include tire coming
off of rim), or brake failure (overheating brakes, brake cable/line failure,
worn pads). Those probably aren't rare enough though...
