and notmuch indexes over and expect things to work?
I have previously tried and some how offlineimap got itself in a tangle
and redownloaded all messages. This is nontrivial as I have about 50g
of mail and indexes.
Any suggestions would be appreciated.

I am still having issues building notmuch from source on OSX.
I'm unfortunately not sure if it is something specific to my setup or
not.
I am running latest git version of notmuch, osx 10.9.
Xapian and talloc are from homebrew.
If I manually edit the makefile to remove -lintl

I made the changes for case insensitivity and I much prefer that.
Perhaps it could be an option if you like it the other way around.

think that enabling the regular expressionsearching is a good idea.

I have recently had to split my mail in to two imap accounts.
Up until now I have been using notmuch with no issues with one account
and offlineimap.
All my email is under ~/MAIL.
I can configure offlineimap with two accounts each in a different
