if I try \setupheader[state=high] (or empty), it works only for the 
first chapter; i.e. at the start of second and every following 
chapter, the header text is visible. Is this intentional?

Yes.

I can work around this with
\setuphead[chapter][before={\setupheader[state=empty]}]

\setuphead[chapter][header=empty] % header=high
