I attached a pdf as an example. However, it does not matter which pdf is used.
If I use the text highlighting annotation, okular will crash after adding 3-8
further high

Yes, when I read my first comment the behavior changed from always single pages
to always duplex. Didn't realize it up to now.
Your modified ppd-file makes no difference here.

The very file existed here, but was empty. I removed it anyway and still,
printing is done with duplex no matter whether "None" or "Long Side" is set
