AFAIK, Okular does stretch/scale pages by default now, so I think the behaviour
is as requested here, s.a. bug 319476. (I just tested with an A5 document and
it was scaled to A4 as expected when using A4 as ouput media size.)

On the contrary, there are requests to make that configurable, but that is
handled in 336617 - so I think this bug can be closed as fixed now.
