Hi Koray,
In 2009 I did an IHE Medical Summary profile based Template for the
Interoperability Demonstration at HIC.  I can't recall the exact
relationships between CCR, CCD and IHE Medical Summary profiles but they
pretty much cover the same concepts.  The template was only a subset of the
Referral Summary but did cover Service request, History of present illness,
Active problems, Current Medications, Allergies, History of past Illness,
Past surgical history, Family history and Social history.

This may be the start you were looking for, or at least something to have a
look at.  Obviously the Archetypes used were from an old point in time on
CKM so I will need to bundle up a package and send it to you separately if
you're interested.  You will then need to upgrade these to the latest
release of archetypes.

You may be interested to know that I used this template to generate a
Template Data Schema (XML Schema) to assist in the transformation of a HL7
V2 REF message sent from another vendor, which was coded using the IHE
Medical Summary profile recommended LONIC codes, into both an openEHR
composition and a Referral Summary CDA.  The CDA document was then sent to
another vendor's XDS.b repository.
