part of the answer is to have what we call an EHR Index service - see in the Service Model overview <https://specifications.openehr.org/releases/SM/latest/openehr_platform.html#_openehr_platform_model>. That is implemented by some of the platform vendors, but not yet officially standardised among them. See section 7 <https://specifications.openehr.org/releases/SM/latest/openehr_platform.html#_ehr_index_package>for more technical detail. The ids stored in this kind of service are normally ones from an MPI, but could be from anywhere.
