It seems that my troubles with pkcs15-init were mostly due to using an old
version of OpenSC. Once I built the new version from source, pkcs15-init asked
for the correct pin.
Old version (0.11.12-1ubuntu3.3 on Ubuntu 10.04):
$ pkcs15-init --erase-card --auth-id 03 --so-pin 87654321
