For me it's more a question of performance than
fragmentation.
At my last client (www.houra.fr), the images were
store at the OS level to speed up the showing of the
web pages and in Oracle so that each managers could
associate comments to the images.

Check out BLOBs. They're stored externally. Can be tough to work with
but also can be very worthwhile.
