A friend sent me this link. There are two pages, and the second page can be
found by moving your mouse around near the center of the right side, in the
black space, to find the arrow to get to the second page that has a less than 3
minute video on folding.
Does anyone know her?

I am usually a lurker but felt like I might be able to help out with this one.
I have the CD that was listed on eBay. There are 990 pages, but no index or
table of contents so no easy way to pass on the names of the creators. The pdf
file is 22,439 KB.
