Just to mention this tool I maintain :
https://github.com/DCbrainOrg/pgsql2osm intended to convert data from pgsql
to osm. Data model is described on readme.
It's currently a draft which would suffer some issues at world scale, but
it brings value on small or medium sized datasets (I currently use it on
whole metropolitan France to run OSRM on custom data).
