We ran into this issue last week out of the blue. We were forced to enabled
"modern auth" for our O365 tenant, and that fixed the issue

Our Minneapolis, Pasadena, and LA Centurylink internet circuits all took a
hit at 17:02 to 17:05 Central.
Zayo seemed to be affected too, but I'm sure it's related to the
Centurylink issue.
