I am looking for some advice regarding the workplace I have just left. We have approx. 130 low risk births a year, in a high risk,remotepopulation. Quite a few women are sent between 1100-3000 kms away to birth, The roster hasbetween 8-10 midwives who work on most shifts with RN and ENs and cover the general ward/mid and ED. I have enjoyed the mid I have had as I get to do the antenatal clinic, classes and births mixed in with general patients.The doctors arevery happy to let midwives be the caregivers for pregnant/labouring women unless we feel there is a need for any intervention. 3 of the doctors are dutch and are very mid orientated. My concerns lie with ( and a big part of the reason that I am leaving) the fact that some of the midwives have very little recency of practise, one midwife has birthed two babies in 6 years with supervision, another has come from years of remote aboriginal community work. We all work most shifts as the only midwife onshift. Management don't have any concerns with this as if they are registered with the board that appears to be enough for them.There have been no performance appraissals of staff in the two years I have been here.I don't believe that the place is safe and there is not enough education to upskill or even to maintain skills unless personnaly motivated.The'near misses' are never reviewed.I could reel off quite a few, let alone some of the things that make my blood boil. I want to write a constructive ,objective exit letter expressing my concerns without shooting myself in the foot for references etc. Should I do that, would it have any use? Has anyone else felt they have been in a similar circumstance? We are moving to south west Vic and building a house in an idyllic settingso I have a lot to look forward to, but would like to leave knowing that something may change. Cath

Hi Miriam, I have done the tiki tour - impressed and would definitely like to mention safetsleep as an option in my book. I do appreciate all the work you are doing and can see some great uses for safetsleep but also have some questions: 1) I would be really concerned that some parents would use Safetsleep as a 'restraint'. But I guess if that were the case, the same thinking would mean that they would not have grasped whatever gentle/ respectful info I had written in the first place so were still coming from a 'convenient baby' / baby as object to be trained mentality. ie -I personally would hate to see such a product used to prevent a toddler from getting of of bed- I have read in at least one book and heard from a MB unit where the treatment for toddlers who dont stay in bed is to remove the lightbulb and lock the door - although I highly doubt you would approve of this either. 2) I am pleased to see that the babies in the letters on your site who had plagiocephaly also received physio -I have concerns that things like helmets on bubs only 'cosmetically' correct the symtoms (ie flat head) not the causes ie the underlying reasons for torticolus/ positional turns ( retained reflexes ? neurology ? tight muscles). I know of several parents here who have had feeding difficulties with such bubs ( ie uncomfortable feeding from one side/ refusal on one side) who have been treated either by a cranial osteopath or a paediatric chiro and when this has been corrected, then these bubs are content to change sides etc (ie the problem is fixed not just the symptom - ie flat head). My own youngest child is regularly treated by a chiro ( initially because of dyslexia which improved remarkably). At the first visit it was found that he still had some early reflexes present and the chiro commented - he would have had difficulty breastfeeding? My response was that as he was my 5th child he wouldnt have had any difficulties - whether this meant holding him upside down if necessary of course i didnt do any such thing but certainly would have compensated for any difficulty by altering feeding positions rather than seeking a cause because at the time i wasnt aware that 'the cause' ie a neurological problem/ retained reflexes could be involved. Incidentally, he didnt/ doesn't have a flat head - he coslept so would have been side sleeping / changing sides perfectly safely and naturally, anyway. (perhaps reinforcing my point that while symptoms can be corrected, this doesnt necessarily mean the child is 'fixed') However, I do have to acknowledge that this isnt every parent's choice and also that sadly, infant sleep is fraught with fear -Im amazed by comments to the mother in the testimonial (on your site) by her plunket nurse re the danger of side sleeping - poor mums. Hi Tania, I did a placement at Kalgoolie hospital18 monthsago for 2 weeks and its comparitively a pretty good place to birth, a great history of low intervention, lowest cs rate in the country one year. There is a birth centre type room, and a few other birthing rooms - and all the midwives were fantastic - of course as in most regional hospitals there was the call the dr when HOV routine,but if things were going well they were happy to keep out..but the antenatal care was done through the GPs. a new Ob was appointed in Jan 2004 so im not sure if his influence has been positive but would be happy to talk to you about my impressions if you wanna give me a ring 82415103 or you have my email address.(or pass on to theother woman) I didn't hear of any homebirth IPM's when i was there. For any interested travelling midwives: i can recommend it as a great place to work and play (lots of pubs!) in the beautiful red desert, and they offer stints for as little as 3 months with excellent accomodation. the midwifery manager is a very supportive woman.
