When you get the "Header already sent" error, that means that you are trying to send header info, e.g. cookie info AFTER you've already sent some header info. This is usually seen when your php script has an include in it (above your cookie stuff) that contains some html. Check your includes, and make sure that the header info is the first thing sent.
