This has been fixed at least in PHP 4.1.0

refiled against 4.0, although with output buffering, it may not be as
important.

It would be really nifty if PHP remembered the file and line of the
first character output when headers are finalized and sent, so that the
Header() and SetCookie() functions could point to a specific place
where the user screwed up when they are called after the headers have
been finalized.

This feature would significantly reduce traffic on the PHP mailing
list.
