I've the same problem with NT4 sp6a IIS.
When i used 4.2.3, i've no problem, but in 4.3.1, i've the problem !
But when i setup IIS to use ISAPI instead CGI, all work fine.

old ext_skel script in php/scripts/ext_skel_ng
ext_skel_ng is meant to replace the current ext_skel completely, and IMHO

compiler and linker settings, i.e. both of them need to have multithreaded
