Bug description: odbc_execute() special quoted parameter handling not documented
If one of the parameters in the array passed to

Use a var to store the page title (use our own page title if we cannot
go up more => first page). This needs some error corrections, but I
would not like to forget to commit

Some problems I encountered, and was unable to solve:
- TOC parts are sometimes tooo dep (especially
on the front page). The two toc depth options does
not seem to have any combination [as I tried] to
print out TOC just as deep as in the DSSSL output.
- The Index appendix
