No have seen it only once. DO you have debugging packages? I could then
reconstruct the line where it was crashing using gdb, the kernel line
and the debugging symbol.

I haven't seen this in our 3 clusters and in 3 training sessions. What
was the hw setup? Did you have any chance to check the logs to check for
errors?
setup was 3 servers, with 2osd
-resize disk qcow2 : not working when snapshot exist (qemu error)
AFAIK qcow2 resize is not possible when snapshots exists. I guess we should
improve
the error message.
