I had simplification turned on, indeed...
I deactivated it :
- in the Options as you mention, so that it is deactivated for newly added
layers
- in my layer options (Rendering properties)
And it is ok now.
It makes sense that if the line is simplified the distance vary !

YES !!! I went back and re-tried several attempts I
had made to get a satellite base map from Mapbox. In every case when I
change the "S" in my Mapbox username from uppercase to lowercase it
works flawlessly.

Do you have geometry simplification turned off? See "Settings" -->
"Options" --> "Rendering" --> "Enable feature simplification ..."
Make sure this is deactivated (I think it is enabled by default).

Quando importi il wms imposta le dimensioni di ogni tassello in x e y.
Prova con 1800 x 1800 e poi prova ad esportare il layout.
Se continui a non vedere, diminuisci le dimensioni, sino a quando non
vedrai lo sfondo anche nel PDF esportato.
