Where are the messages originating from?
(Check the message headers)
Either a spammer is forging the address, or it's self inflicted from
somewhere else, like a web app of some sort. I would guess the later.

Is the account listed in qmailadmin?
If so, how did you create the account? If you used vqadmin, perhaps
there's yet a bug in that program when creating users.
