You may be looking for qvm-usb.  Run in dom0 with no arguments.  The first 
column is the device name that you supply next to "qvm-usb -a $VM $DEVICE".

PS even if you made a mistake and lost mouse and keyboard, couldn't you power 
cycle and get them back?
