Those errors are caused by an entry missing from your dictionary. I would guess
you are using dictionary.ascend?

You need to add these to your dictionary:

ATTRIBUTE       NAS-Identifier          32      string
ATTRIBUTE       Proxy-State             33      binary

Hmmm, its a bit hard to tell, but I think that maybe the mysql client libraries
(part of the mysql package, not DBD-mysql) are not installed, or not installed
in the place that DBD-mysql expects them to be?
