I would say they run normal. I've found that as I age my feet have slightly grown and I now am on the cusp between 44 and 45 for most shoes, while I used to be a size smaller. If you are happy with Shimano 45s, I'd be inclined to think that 45 Exustars would work, too. My 44s feel much the same as
