Sorry for the delay in getting back to you on this!

The "published" event is used any time a draft of a review request is
published. This means the first time it's been published, and every time a
change is made or new diff is uploaded and then published. Basically, any
time the "Publish" button is pressed.

There's no way today to do it on updates. The handler on the other end will
have to check for this. You can use the "is_new" field in the payload to
determine this.
