I've got the Dvorak layout working with my SAM... I don't know if
anyone here uses Dvorak so I thought I'd just post it.
A few problems:
* Since the SAM has its quote characters on Shift-7 and next to
return, I'm not sure what should go where 'Q' used to be.

I have used a few extra keys on the PC Keyboard - some examples for you:
NumLock triggers SHIFT-EDIT on the Sam, which toggles the function keys to
numbers and vice versa.

WINDOWS MENU KEY is EDIT,
both ALT's are Symbol,
and CTRL maps directly to Sam's CNTRL key.
I've also got F11 generating a clean NMI and F12 is RESET.
