I guess it didn't fix it after all. I switched over to memcached and
the problem is still there, only with the shipping settings -
everything else is fine. I fired up the sample "large" project and it
works correctly so it must be a configuration on my end. Still looking
into it...

It could be your caching settings.  If you are using filecache, then
it can be that the user which is running the store doesn't have
permission to the cache directory.  This can happen if you run it once
as root and then try to run it again as a non-root user.
