Re: [Sepsis Groups] Core Measures for Sepsis

2017-09-11 Thread Nielsenshultz, Yara
Because it's not yet directly tied to reimbursement, we've started looking at
sepsis mortality rates and plan to look at overall length of stay and ICU
length of stay in relation to bundle compliance. We can then consider the
financial impact of those factors. Reducing LOS (appropriately, of course) may
also affect Medicare Spending per Beneficiary (MSPB), although I'm not sure how
you would quantify the impact.

Here are some resources I've used:

* Proportion and Cost of Unplanned 30-Day Readmissions After Sepsis
Compared with Other Medical Conditions (Feb 2017) -
https://www.ncbi.nlm.nih.gov/pubmed/28114505

* Time to Treatment and Mortality during Mandated Emergency Care for
Sepsis (May 2017) - http://www.nejm.org/doi/full/10.1056/NEJMoa1703058

* Creating the Best Medical Result at the Lowest Necessary Cost (Mar
2016) -
https://intermountainhealthcare.org/-/media/files/healthy-dialogues/james-slides-2016.pdf?la=en
 (Starts at p. 15)

I don't know of any source for reimbursement based on the sepsis core measures
compliance. It isn't tied to value based purchasing yet.

Hello,
Does anyone know where I can find the details for Core Measure for Sepsis? Not
what they are-but the threshold for payment? Is it just SEP-1 or is each
element measured for compliance & payment? What percentage is required? I
thought it might be in the 5.3 version-but cannot find the specifics for
reimbursement. Any information you can share would be appreciated. Our
compliance rate is just over 50% & am planning on next steps to improve our
numbers. Knowing the financial impact will help gain support. Thank you-

Agree. I assumed presence of infection.

Is there a source of infection documented? Or documentation of severe sepsis
or septic shock? If not, then I disagree.

I would say yes. A lactate >/= 4 is automatically septic shock (as long as the
patient also meets criteria for severe sepsis, which it looks like they do).
