Speaking of the internal documentation, there is mention in that PDF
of certain routines being useful for a GUI based simulator.
Has anyone used SIMH's guts to create a GUI based simulator?
If so, is it open source so I can look at the implementation?

If you are seeing Japanese text, it's because the primary API set in
VS changed from ANSI to Unicode...
Look for all the api calls and either put A's on the end of them
(MsgBoxA, CreateWindowA..), or there is some macro to set, or a flag
to the C compiler to default to ASCII
