I can't see any indicators in the logs, but would welcome an opinion.

Check the apache logs - that will probably point you in the right direction.
If it turns out, it's a general problem and most people will have it in Ubuntu
14.04, posting follow-up to the list [describing the problem and the
work-around/fix] would be great. I can try to get it into the Wiki/Howt
