I don't think we should drop ext.def.fl (the name can change, but the 
functionality is useful)and am going to reopen this.  Namely, it is often the 
case where one wants all values that aren't explicitly mapped to go into a 
default field.  Since all metadata fields aren't knowable up front, there is 
currently no way to express this in the ExtractingRequestHandler.
