I thought the rule was that fields in a select statement with a group by had
to either be in the group by or be an aggregate function, e.g
select a, b, count(*)
from some_table
group by a
should be an error. If not what value does the "b" field have?

Vacuuming is just slow. I don't think there is much you can do except
don't do it unless you really need it, and don't turn on autovacuum.
