squid-3 memory leaks

HI,

I've fixed, hopefully, a large chunk of the memory leaks people
have been seeing in squid-3-cvs. I suggest upgrading any installs
you have to the latest and give it a run.

I'm currently stressing out squid-3-cvs with epoll and diskd.
