Thanks for your reply. I checked all of these. I reset passwords, set
the perms on proxyauth for everyone, and double checked proxyauth, copied
from another server that does work, and it did not make a difference.

Try login with this username+passwsord. Maybe system asks you to change the
password.
(2) this username may is denied to read proxyauth
(3) content of proxyauth is not exactly "allow"
