If the value in the config.php file is 2, then STARTTLS is enabled.

I has checked the config.php file, and here the value already is set to "2" via 
the administrator plugin from version 1.5.23.

Actually, the Administrator plugin for 1.5.2 did have STARTTLS support
already, but it was slightly broken.  I fixed that and added full
STARTTLS support for the plugin under 1.4.23.  Snapshots with said fixes
should be available in the next daily SVN snapshot on our downloads page
within 24 hours or via SVN directly now.

The administrator plugin was never updated with this selection when
STARTTLS was added.  I will make a fix in SVN in the coming days.  In
the meantime, you can directly edit config/config.php

You seem to understand STARTTLS well enough but note that the
SquirrelMail $use_smtp_tls setting can have three values: off (0), TLS
(1), and STARTTLS (2).

SquirrelMail supports STARTTLS.  If I recall correctly, you set
$use_smtp_tls to 2 in config.php

If your hosting company provides services on port 587, then you can telnet
to it.

Are you sure it requires a TLS connection from the outset? Normally port
587 is used with STARTTLS, where the connection is plaintext and then
upgraded. Sometimes you can connect to port 465 with TLS.
