On that specific question, I've seen plenty of places where an imported GB boundary nearly-but-not-quite matches a GPS-, Bing- or OSSV-derived river. I'm guessing that the river's moved since boundary was drawn.

Are inland GB bundaries ever defined in terms of something else (e.g. a river or stream) or once defined do they tend to stay where they are or move slowly as the river changes course?

When adding streams where boundaries have already been imported I've been reluctant to use the boundary way as the stream course because there are often clear places where it's not correct. I wouldn't want to alter the location of the boundary way of course, as the boundary itself is usually not marked on the ground.

With bigger rivers it's different I guess - the Thames isn't going to be allowed to cut through Canary Wharf and create an oxbow lake between Greenwich and Millwall any time soon!
