postgresql wasn't running. Not sure if that was intended or not (logs
indicate the last thing was a fast shutdown which suggests human action).
I've restarted it and will monitor for updates in the services that depend
on it (e.g. osm2pgsql, nominatim, etc)

Thanks to Donal Diamond, I have login access to the servers and have
started diagnosing the issues with tileservers updating.
Not sure where the best place to track state of issues is (Is there a bug
tracker somewhere that you use?).
Anyway - looks like a protgresql unix domain socket issue.
Donal - nice will chat on irc
overpass.ie is getting lots of requests from 2 ips in Karlsruhe and
diskspace filled up due to logging.
I know one OSMIE member currently living in Karlsruhe ;-)
