Hi, I suppose that your pdftex (0.13d) is much too old.
The current beta release is 0.14g and can be found at

ftp://ftp.fi.muni.cz/pub/tex/local/cstug/thanh/pdftex/snapshots/2912/

You don't have to compile everything (which didn't work for me), just
./configure && make pdftex.  And please don't forget to replace the
old pdftex.pool as well.

You might also need the current version of pdftex.def.

I am having difficulty including JPEG images - I am running teTeX-1.0.7
(I think its
the latest version).  I have attached the log file.  It runs fine on my
friend's teTeX version which is
older than my version (and hence puzzling).

A consistently organized package such as teTeX makes it much easier 
to create add-on packages that will install easily.  It is better
for maintainers to concentrate on core functions.  

Where things get a bit complicated is keeping track of free clones
of commercial packages like MathTime.  Similar issues occur for
the URW clones of Adobe Type1 fonts.  I would like to see more
explicit indications when such substitutions occur, for example,
a MathTime package with a Belleek option (e.g.,
\usepackage[belleek]{mathtime} or \usepackage[urw]{pslatex}).
