I only ever used it directly in the metapost (it's in my code for
creating custom labels). In the .th, Therion turns everything into
\char103 escape sequences. In the metapost, you have more control.

I can make the same effect using the tex parameter \medskip as demonstrated
in the Therion \legendcontent routine.
But sometimes it would be nice to do it in drawing labels and in strings
