You are using InlineJavascriptPlugin v1.8.0 (which is over 3 years
old!!). The current version is 1.9.6
One of the changes (see v1.9.0) was to automatically add 'return
false' handling to onclick scripts. This prevents the double-
invocation

problem using your script, which suggests the issue is not with the script.
Do you happen to have this script in ProblemTemplate4? If you do, make sure
