What kind of server are you using and is the server possibly behind a
reverse proxy?
If you are behind a reverse proxy, you are probably missing a rewrite rule
to change the Destination header in the http request, please see the
following two links for suitable reverse proxy configurations for IIS and
Apache httpd:
