Summary: After plug and unplug headphone, the internal speaker has no sound
output.
Manually clicking output device again does not recover.
Only reboot could recover from this issue.
Reproduce steps:
1. Plug headphone
2. Unplug headphone
3. Check
I have the same issue on Disco,

In Ubuntu 16.04, camera-app (version 3.0.0+16.04.20160323.3-0ubuntu1)
cannot preview webcam or capture anything, the camera-app window is all
black.

With latest image,
I cannot reproduce this issue, after unplug USB keyboard, internal virtual
keyboard is working well.

I could also reproduce this issue with Mako, using Vivid r88
Pulseaudio version: 1:4.0-0ubuntu23
The pulseaudio is still holding 6% CPU usage even switch to other
application.
