Additional tests done on bionic: after changing the parameters set in
/etc/security/faillock.conf to:
deny=2
unlock_time=20
By trying to authenticate with the wrong password 2 times, it was
verified that the account was locked for the amount of time set to the
unlock_time parameter (20s).

Test consisted on setting up pam_faillock with the following
configuration, as described in the man page:

Everything works well, except the Headphone jack. The Headphones are
detected, they are not mutted in alsamixer and pavucontrol detects them
and detects sound is going to the headphones. However there is no sound.
With speakers everything is allright

Reviewed patches and they look good to me.
However, in the future, we should consider another possibility: disable
FIPS mode for libNSS3 by default, since that lib isn't FIPS-certified.
This can prevent customers from mistakenly think the opposite.

After installing pulse audio equalizer, it worked good. everything was
detected in Sound Definitions and there was sound in the headphone out.
In the day after, every input and output devices were gone in Sound

hotspot the device power off and it wont power up anymore. Recovery mode is not

I have a similar problem using qemu-kvm in kubuntu 14.04.1, while running
linux-mint 17.1 or elementary os 0.3 beta 1 in the virtual machine.
In the case of linux mint, the desktop is more or less ok, but some objects are
corrupted.
