One way to reproduce:
* open terminal
* tap on Ctrl+D
Terminal's gone, but ghost remains. It seems either of upstart, ubuntu-
app-launch, application manager don't notice the app gone away.

I see this sometimes too.
I had a couple of apps ghosted on arale latest rc-proposed image.
If I tap them they come to the front but transparent, like the screenshot is
missing.
After a few seconds the app wakes up again (is restarted) and it
appears.
